If your employer or school is requesting a Med3 or private letter from your GP because you are having to isolate please see standardised template below to give to them. A sick note (Med3) will not be issued for isolation as this is a not sickness but a precaution to avoid transmission of any illness that is yet to produce symptoms.

If you do require private letters or need to request medical notes, please complete one of the consent forms below and email to OLDCCG.p85012@nhs.net using your own email. Please do not bring paper copies to the Practice as they will NOT be accepted. Due to the Covid-19 situation, unfortunately we cannot give you a time frame for when requests will be actioned by. If you are requesting a private letter or medical notes on behalf of a family member please ask them to complete the 3rd party consent form attached below along with the relevant consent form.

APPOINTMENT INFORMATION 

The surgery operates an advanced access appointments system. This means that we allow pre-booking appointments by up to 4 weeks and a certain amount of appointments are saved for patients that ring “on the day“ for emergencies. We aim to be able to offer an appointment within 24 hours but our reception staff who are trained as care navigators will need to ask a few questions to be able to determine if you require a GP appointment or, if your symptoms could be dealt with by other services such as pharmacy. If if you wish to specify a particular Doctor, you may need to wait longer as their appointments may not always be available due to demands. Please note the surgery offers same day appointments for all under 5 year old children if it is clinically indicated. 

RECEPTION TIMES: PLEASE NOTE THAT ALTHOUGH THE RECEPTION WILL BE OPEN EARLY IN THE MORNING OUR PHONES LINES ARE NOT AVAILABLE UNTIL 8.00AM AND THEY CLOSE AT 6.30PM IN THE EVENING.
